Eligibility

Eligibility Criteria

To be eligible for funding and assistance from Legacy Lifeline Foundation or any partners in our network (See Partners and Providers), individuals must meet the following criteria:

  1. Age Requirement: Must be 18 years of age or older.

  2. Citizenship: Must provide proof of American citizenship.

  3. Residency: Must be a resident of any of the 50 United States.

  4. Mental Health Need: Must demonstrate a need for mental health support or substance use disorder treatment.

  5. Film Screening Attendance: Must have attended an official Legacy Lifeline Foundation film screening or participated in an approved mental health program.

Services Offered

Legacy Lifeline Foundation provides the following services to eligible individuals:

  1. Mental Health Counseling: Up to three sessions with approved digital mental health providers.

  2. Resource Navigation: Assistance in finding short-term mental health resources and support.

  3. Educational Programs: Access to mental health workshops and educational materials.

  5. Data Retention: Personal information is retained only for the duration necessary to provide services and comply with legal requirements.

  6. Access to Records: Individuals have the right to access their personal records and request corrections if needed.


Application Process

To apply for assistance from Legacy Lifeline Foundation:

  1. Attend an official Legacy Lifeline Foundation-approved film screening or participate in a partner mental health program.

  2. Complete the online 5-minute self check, providing necessary documentation to prove eligibility.

  3. Undergo a brief mental clarity check to assess possible mental health needs and get you funded.

  4. Receive information about available services and next steps for accessing support.

Our goal is to ensure that individuals receive the treatment, diagnoses, and care they need to become the best possible versions of themselves, regardless of their occupation or background.
